End-of-life care and support for liver disease

When advanced liver disease changes what each day looks like, you do not have to navigate the next steps alone. The Liver Hope Foundation offers compassionate, non-clinical guidance for patients, families, friends, and caregivers in Henderson, Nevada, and beyond—helping you find understandable information, emotional support, and trusted community resources alongside your medical team.

Support services for patients and families

Our services are designed to reduce isolation and make difficult conversations, decisions, and day-to-day responsibilities feel more manageable. We do not provide medical treatment, prescribe medication, or manage pain directly. Instead, we help you prepare to speak with your clinical team and connect with appropriate palliative-care, hospice, counseling, and local support resources.

Education and care planning

Receive clear education about advanced liver disease, treatment options, and questions to bring to your medical team. We help families feel more prepared for conversations about care preferences and next steps.

Emotional and caregiver support

Find a welcoming space for questions, peer connection, and practical support. Our caregiver and community programs can help families manage stress, reduce isolation, and feel less alone.

Resource navigation and referrals

We connect families with community services, respite-care information, transportation, and financial-assistance resources when available, as well as appropriate palliative-care, hospice, and counseling referrals.

A compassionate partner alongside your medical team

Medical professionals focus on clinical care. The Liver Hope Foundation complements that care by translating complex information into practical guidance, creating space for emotional support, and helping families locate community resources. Patients, caregivers, loved ones, and healthcare professionals seeking a referral resource are welcome to connect with us.

Help with the conversations that matter

Facing end-of-life decisions can bring uncertainty, grief, and many unanswered questions. We can help you organize concerns for your healthcare team, understand when it may be time to ask about palliative care or hospice, and identify resources that fit your family’s circumstances. For urgent or life-threatening symptoms, contact your clinical team or call 911.

Begin with one conversation

Tell us what you and your family are facing. We will listen and help connect you with educational resources, caregiver support, community-based services, transportation options, and financial-assistance information when available. Our goal is to help you feel informed, supported, and better able to move forward together.
